3. your physical mailing address (include postal zone)
4. date you were born
5. a totally blank (no "pay-to" name) postal money for $200

For UCC filings:

1. a certified copy of your birth certificate, and your children
2. your full "sovereign" name (Ex: John-Edward:Smith)
3. your physical mailing address (include postal zone)
4. date you were born
5. a totally blank (no "pay-to" name) postal money for $200
6. daytime phone number
7. your email address
8. your social security number, and your children
9. driver's license number and state
10. your auto VIN number, make, model and year
11. your full "strawman" name (Ex: JOHN EDWARD SMITH)
12. what States you have resided in the last 5 years
    (NOTE: I will provide a UCC-11 for each state, but you will
    have to file these and send in proper fee - about $25/state)

Once I get this info and donations, I will file your UCC-11 first, to see if you have any outstanding UCC-1 filings against you. If you have any, you must clear these first so you will have clear title to your strawman.
